Notes from the CMS Nurse
From the CMS Nurses:
If you have not done so yet, please forward the following documents to the nurse's office or upload them to the SNAP portal by December 1, 2025:
*6th grade: Proof of physical exam dated after July 1, 2024.
Please include an updated immunization record.
(This 6th-grade physical is the perfect time to think ahead to 7th-grade immunization requirements. Tetanus, diphtheria, pertussis (Tdap), and meningococcal (MCV4) immunization proof must be sent to the nurse’s office before August 24, 2026, for your child to be able to start 7th grade on time. This can be completed now.)
*7th grade: Proof of dental exam dated after July 1, 2024.
A community dentist will visit CMS in the Spring of 2026 to conduct exams on children who have not submitted a form from their own dentist.
Blank forms for both exams can be found on the School District website, www.colonialsd.org under Parents/Guardians —> Forms and Documents —> School Health Services. Your provider’s version of the form is also acceptable.
